| The Intensive Family
Preservation Program and the Family Reunification Program are two of the
many programs within the Community Child Guidance Clinic that provide
home-based therapeutic services to families. All referrals to the
programs come through the Department of Children and Families.
The primary goal of the Intensive Family
Preservation Program is to maintain children and adolescents in their homes
and communities, thereby preventing out-of-home placements. Criteria
for referral include that the child be in imminent risk of out-of-home
placement. The services are provided for up to 12 weeks.
The primary goal of the Family
Reunification Program is to reconnect children and adolescents who are in
out-of-home care, (either residential or foster care), with their families.
Service begins approximately 4 to 6 weeks prior to discharge and continues
for up to 16 weeks.

possible barriers to reunification |

Collaborative consultation
with schools, other professionals and community agencies |

Connection of the child and
family to appropriate community resources |

Develop a safety plan for
anticipated crises |
Professional Staff
The Family Preservation and Family
Reunification workers are Masters-level Clinicians with training and
experience in providing home-based services.
